Urinothorax is an extremely rare entity characterized by the presence of urine in the pleural space. It can occur as a complication of urological surgical procedures or post-renal obstructive conditions or abdominal trauma. Urinothorax patients often present with respiratory symptoms such as cough, shortness of breath, or chest pain. A 52-year-old woman was admitted to the emergency department with shortness of breath. Her medical history included nephrolithotomy 10 days ago. Chest X-ray and computed tomography revealed massive right pleural effusion. Thoracentesis was performed and the rate of pleural fluid/serum creatinine level was found to be higher (2.05). The patient was, then, operated with the diagnosis of urinothorax and tube thoracostomy and ureteral stent intervention procedures were performed. She was discharged in the postoperative fifth day.
