Nokardiyoz, olduka nadir grlen ve zellikle immnspresif olgularda ortaya kan frsat bir enfeksiyondur. Ancak enfekte olan olgularn yaklak te birinin immn sisteminin normal olduu bilinmektedir. Nokardiyozun klinik tablosu ok geni bir yelpazede olumakta ve bu nedenle dier enfeksiyonlar hatta maligniteler ile bile karabilmektedir. Patognomonik bulgusu, semptom ve radyolojik zellikleri olmayan bu hastaln tans bazen g olabilmektedir. Herhangi bir immnspresif hastalk ya da tedavi yks olmayan olgumuz, nefes darl, halsizlik ve ate yaknmalar ile bavurdu. Kliniimize pnmoni ve parapnmonik efz-yon n tanlar ile yatrld. Balangta non-spesifik antibiyotik tedaviye yant vermesine ramen, tekrarlayan pnmoni, beraberinde sa kolda vena sefalica ve vena basilicada trombs ile seyreden pulmoner nokardiyazis olgusu, nadir grlmesi ve tan koymadaki zorluklar nedeni ile sunulmutur.

Pulmonary Nocardiosis: An Unusual Cause of Pulmonary Cavitations

Nocardiosis is an uncommon opportunistic infection which particularly seen in immunosuppressed patients, although approximately one-third of infected patients are immunocompetent. It has a broad range of clinical presentations and may present similarly to several other infections and malignancies. The diagnosis can be challenging due to the lack of pathognomonic signs, symptoms, and radiological findings. Herein, we report a rare case of pulmonary nocardiosis in a patient who had a thrombus in the right arm vena cephalica and vena basilica of the right arm and was admitted with dyspnea, weakness, and fever without any immunosuppressive disease or treatment. The patient was hospitalized with a preliminary diagnosis of pneumonia and parapneumonic effusion, and responded to antibiotherapy.
